Whether you are an experienced home health clinician or looking to transition into home health, we would love to speak with you.
What You’ll Do:
- Perform speech therapy evaluations and reassessments in patients’ homes.
- Develop individualized treatment plans based on physician orders.
- Treat speech, language, cognitive, communication, and swallowing disorders.
- Educate patients and caregivers on therapy techniques and safety strategies.
- Collaborate with nurses, therapists, physicians, and interdisciplinary team members.
- Complete accurate and timely documentation in compliance with Medicare and Arizona regulations.
- Participate in case conferences as needed.

Qualifications:
- Current Arizona Speech Language Pathologist license in good standing.
- Master’s degree in Speech Language Pathology.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong communication and documentation skills.
- Home health experience preferred, but not required.
