Brighter Days Home Health is seeking an experienced and dependable Registered Nurse (RN) to join our growing home health team on a PRN basis.

Responsibilities:
- Perform skilled nursing visits in accordance with physician orders and the plan of care.
- Complete patient assessments, admissions, recertifications, resumptions of care, and discharges as assigned.
- Administer medications, wound care, and treatments.
- Monitor patient condition and communicate changes appropriately.
- Educate patients and caregivers regarding medications, disease processes, safety, and ongoing care needs.
- Accurately complete documentation in compliance with agency policies and Medicare Conditions of Participation (CoPs).
- Coordinate care with physicians, therapists, case managers, and interdisciplinary team members.
- Participate in case conferences and care coordination activities.
Qualifications:
- Current Arizona RN license in good standing.
- Graduate of an accredited nursing program.
- Minimum of 1 year nursing experience required.
- Home health experience preferred.
- Oasis documentation experience preferred.
- Current CPR certification.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Strong clinical judgment, communication, and organizational skills.
- Ability to work independently and manage time effectively.

Compensation:
- Competitive per-visit rates based on visit type and experience.
- Additional compensation may be available for admissions, high-acuity visits, extended travel areas, and weekends.
