Responsibilities:
- Perform initial evaluations and ongoing reassessments in accordance with physician orders.
- Develop individualized treatment plans focused on safety, mobility, strength, balance, and independence.
- Provide therapeutic exercises, gait training, transfer training, and patient/caregiver education.
- Accurately document visits in compliance with agency policies and Medicare guidelines.
- Collaborate with nursing and interdisciplinary team members to coordinate patient care.
- Participate in case conferences as needed.
- Identify changes in patient condition and communicate appropriately.
Qualifications:
- Current Arizona Physical Therapist license in good standing.
- Graduate of an accredited Physical Therapy program.
- Minimum 1 year PT experience preferred.
- Home health experience strongly preferred.
- Current CPR certification.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Strong communication and documentation skills.
- Ability to work independently and efficiently.

Compensation:
- Competitive per-visit pay.
- Additional compensation may be available for extended travel areas and weekend visits.
Service Areas: Phoenix, Glendale, Peoria, Surprise, Scottsdale, Mesa, Chandler, Tempe, Avondale, Goodyear, and surrounding areas.
