Brighter Days Home Health is seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team on a PRN basis.

Responsibilities:
- Perform occupational therapy evaluations and ongoing reassessments in accordance with physician orders.
- Develop individualized treatment plans focused on improving safety, independence, and quality of life.
- Provide interventions related to activities of daily living (ADLs), mobility, upper extremity function, cognitive skills, energy conservation, and home safety.
- Educate patients and caregivers on adaptive equipment, fall prevention, and functional independence.
- Accurately complete documentation in compliance with agency policies and Medicare guidelines.
- Collaborate with nursing staff, therapists, physicians, and interdisciplinary team members.
- Participate in care coordination and case conferences as needed.
- Monitor patient progress and communicate changes appropriately.
Qualifications:
- Current Arizona Occupational Therapist license in good standing.
- Graduate of an accredited Occupational Therapy program.
- Minimum 1 year OT experience preferred.
- Home health experience preferred.
- Current CPR certification.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Strong communication, organizational, and documentation skills.
- Ability to work independently and efficiently.

Compensation:
- Competitive per-visit pay based on visit type and experience.
- Additional compensation may be available for extended travel areas and weekend visits.
