At Brighter Days Home Health, we understand that healing involves more than medical care alone. We are seeking a compassionate and dependable Medical Social Worker (MSW) to join our growing home health team on a PRN basis.
This role is ideal for someone who is passionate about helping patients and families navigate difficult life transitions while maintaining dignity, independence, and access to essential resources.

What You’ll Do:
- Complete psychosocial assessments in patients’ homes.
- Assist patients and families with emotional support, coping strategies, and community resources.
- Help coordinate financial, social, and community services.
- Provide counseling and support related to illness, disability, adjustment, and long-term planning.
- Collaborate closely with nurses, therapists, physicians, and interdisciplinary team members.
- Participate in discharge planning and case conferences as needed.
- Maintain timely and accurate documentation in compliance with agency policies and Medicare regulations

Qualifications:
- Master’s Degree in Social Work (MSW) from an accredited program.
- Current Arizona licensure/certification as required.
- Minimum of one year healthcare experience preferred.
- Home health experience a plus, but not required.
- Excellent communication and interpersonal skills.
- Strong organizational and documentation abilities.
- Valid driver’s license, reliable transportation, and auto insurance.
