Job Summary:
Brighter Days Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing home health team on a PRN basis. This position offers flexibility while allowing you to provide meaningful one-on-one care to patients in the comfort of their homes.
We are looking for clinicians who are reliable, organized, patient-focused, and passionate about delivering high-quality care.
Responsibilities:
- Provide skilled nursing visits in accordance with physician orders and the plan of care.
- Perform patient assessments and ongoing nursing evaluations.
- Administer medications and treatments as ordered.
- Monitor patient condition and report changes appropriately.
- Educate patients and caregivers regarding disease processes, medications, and safety.
- Accurately complete documentation in compliance with agency policies and Medicare guidelines.
- Collaborate with physicians, therapists, case managers, and interdisciplinary team members.
- Participate in case conferences as needed.
Qualifications:
- Current Arizona LPN license in good standing.
- Current CPR certification.
- Minimum of 1 year nursing experience preferred.
- Home health experience preferred but not required.
- Strong communication and organizational skills.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Ability to work independently and manage time effectively.
